DIRECTIONS TO THE STUDENT:

2. Give this form to your teachers before class on Thursday or no later than Friday.
3. Ask each of your teachers to provide the requested information.
4. Over the weekend, take the form home for parents to look over and sign. (PARENTS: Please call the classroom teacher or counselor at FVL during the school day 739.4441 should you have any questions or concerns.)
5. The student is to return this form to his/her counselor on Monday signed by a parent.
